Interestingly, but not surprisingly, much of the Botox information on the web has plastic surgery interests. There are quite a few sites that are location/city specific for centers that use BOTOX. Depending on where you live, search Botox AND cerebral palsy. I found quite a few hospitals advertising clinics that provide Botox. The sites do not say much more about how Botox works than our paragraph in "other options". (Refer to that area for a very basic description of how Botox works). They do let you know where to find it, however. The patient should also be sure to ask his/her current orthopedic surgeon or physiatrist if they offer it.

Safety is really not a current issue with Botox. The main draw back is the expense, the fact that it may lose effectiveness with repeat injections and the fact that it tends to wear off in three months. Some studies claim longer lasting effects in the lower extremity with concurrent casting/bracing (casting at the same time).There are some new UE studies out, so we will have to see how they unfold.

As for expense, different areas and insurance companies will differ in the coverage. As of my last check in NY, Medicaid did NOT cover it, but some centers provided it pre-bono. You may want to call your local large university based medical center to see if they have a CP, neurology or spasticity clinic that offers it.

We use Botox in the upper extremity in a limited manner to simulate surgery or to decrease tone temporarily before or after surgery. I have had patients very happy with its use in the lower extremity. We'd love to hear from patients/families with their thoughts on Botox. CB
